- Extensions modal now shows clearer enable/disable and install hints that match what each key actually does

- Folder trust no longer prompts for or scans the entire home directory when it is a git repo
- Code blocks no longer lose their background shading on the final line of unterminated fences
- Plan mode now activates immediately when toggled during an active turn instead of waiting for the next prompt
